Alaska, Hawaii, Minnesota, New Jersey, New York, North Dakota, South Dakota, and Vermont: Insurance plans are not available in your state.
Massachusetts: Association membership is required for access to Mid-West’s association group health insurance plans (Forms MW-25906-C MA (7/07), MW-25939-C MA (7/07), and MW-SG524-4-7/07) that are available to eligible individuals and small employer groups with one to five employees. Association membership is not required for access to [MEGA’s/Mid-West’s] small employer plans (Forms MW-25906-C MA (7/07), MW-25939-C MA (7/07), and MW-SG524-4-7/07) that are available to small employer groups with six or more employees.
Washington: You must be a part of a group to be eligible for this offer. All Eligible Employees of the Employer will be eligible for health insurance under the Employer’s membership in the Association. Optional benefits are not available in WA.
North Carolina : Does not offer Vision Insurance.
Indiana: Does not offer Dental or Life.
Return of Premium Option: Form #25044, or your states variation. Health insurance premiums returned at age 65, less any claims paid to you. Not available in AR, CT, GA, MA, TX and WA.
Dental: Not Available in IN and WA. Form #25879-P, or your states variation. $50 Deductible per insured per calendar year, $1,000 Calendar Year Maximum per insured, per calendar year. Orthodontia rider (Form #25880) is available ONLY for insured persons under age 19, $50 per month benefit maximum per insured person, $1,000 per insured person lifetime maximum benefit, 12 month waiting period.
Universal Life Insurance: Cash accumulation is dependent upon premiums paid, interest credited, administrative fees and cost of insurance. Policy may terminate should cash accumulation not be sufficient to cover charges and fees due Consult a tax advisor for your particular situation.
Vision Insurance : Form #25213-P, or your states variation. One free exam per 12 month period for a prescription for eyeglass lenses. Contact lens examination will require additional fees.
